STEP 1 – Print the Forms

get adobe readerAt least 24 hours prior to your appointment print the forms below. If the time stamp on the consent forms is not at least 24 hours prior to your appointment, you will be forced to reschedule. STEP 2 – Verify the Forms

After printing out the forms above, verify that the time and date are correct. Look on pages 3, 4, and 7 of the Consent Forms Package and make sure the time is current. If not, the form will not be accepted. Re-open the PDF and re-print the forms. If the forms do not have the correct date and time on them, you’ll be required to reschedule your appointment. If you are unable to print the forms with the correct date and time on them, please contact the office at (913) 491-6878.

STEP 3 – Provide Patient History and Registration

Once you have printed valid copies of the Consent Forms package, please continue to the Center for Women’s Health patient portal and fill out your patient registration and history. Without the completed patient registration and history we will be required to reschedule your appointment. Click the below link to fill out your patient registration and history.

STEP 4 – Ensure You Have All Forms

In addition to the Consent Form you printed out If you are a minor, please refer to the following page and make sure you have the appropriate paperwork. Minors Seeking Abortion.
