ATTENTION: MINORS
Effective July 1, 2011, Kansas laws require that BOTH parents must be present at our office to consent for their minor daughter to have an abortion.
If only ONE of your parents will be with you for your appointment, then you must PRINT OUT the next document (“Minor Consent Form“).
The parent who will NOT be with you must sign the document in front of a Notary Public. Bring the Notarized document with you.
If BOTH parents are with you, then both parents (and, you) may sign a copy at our office, and we will notarize here at no charge.
